diagonal fold lines from the center fold intersections on both the left and right vertical fold lines up to the top edge and also from the top and center fold intersections across to the side edges (where it has been folded out of alignment)

Large strip of tape on both top corners (largest is 5 x 3 cm and the adhesive has stained the paper beneath on both)

1 cm tear and two extremely tiny tears along with mild creasing/edge wear on the left edge
